- Birth certificate, 1 copy
- Housing registration, 1 copy
- Parent’s identification card, 1 copy
- Child’s photograph, one inch size, 4 copies.
- Parent’s photograph, one inch size, 2 copies.
- Child’s health examination certificate, 1 copy
